All Equifax codes were emailed to individuals affected by this privacy breach on July 17. The codes will be mailed by regular mail to the parents that did not have an email address in our system on Wednesday, July 22.

Acknowledgement and Confirmation Form to Complete
Please complete and submit the as soon as possible. The information collected through these forms is an important part of our mitigation efforts and helps us verify that the email and attachment have been deleted and not shared further. A separate form must be completed by everyone who received the email. If more than one parent or guardian received the email, each recipient must complete and submit their own form.
Credit Protection and Identity Theft Services
The SWLSB will be covering the costs of credit monitoring and identity theft protection for a period of three years, to all individuals affected by the privacy breach incident at Rosemere High School, which will notify you of any unusual activity related to your credit. We are currently finalizing arrangements with Equifax. Additional information and registration instructions will be sent to you by email. No action is required on your part at this time regarding the credit protection service.
Recommended precaution measures
- Place a free credit lock on your credit file with and . This prevents financial institutions from accessing your credit file to approve new credit applications, making it more difficult for someone to open credit in your name.
- Monitor your accounts and credit card statements for any unusual activity.

- Be cautious when sharing personal information, whether by telephone, email, on websites, or through social media. Never respond to unsolicited requests for personal information.
- Avoid clicking on links or downloading attachments contained in suspicious emails.
- Change your passwords regularly and ensure they are strong and secure. Never reuse the same password across multiple accounts.
- Review your personal information regularly with organizations that provide you with services and monitor your bank and credit accounts. If you have concerns or identify any suspicious activity, report the incident to the appropriate authorities.
